The notice shall include a copy of the decedent\u2019s death certificate. The notice shall be given as provided in Section 1215, addressed to the director at the Sacramento office of the director.<\/p>\n<hr \/>\n<p><strong>History<\/strong><\/p>\n<p>Added Stats 1990 ch 79 \u00a7 14 (AB 759), operative July 1, 1991.
